Minimum Sign-up Period *

I understand that I am committing to use and pay for weekly service for at least 3 months/12 weeks.

Your service confirmation and initial appointment is dependent upon agreeing to pay for at least 3 months/12 weeks of service. If the service is stopped prior to the agreed upon period, then the account will still be charged for any remaining weeks or months to fulfill the minimum requirements.

Reservation policy *

I understand that I will need to call in a credit-card within 48 hours of receiving my confirmation email.

You will receive an email confirmation with your scheduled appointment date within one business day after our office receives this online registration form. Once you have received your confirmation email from us with your scheduled date of service, please call our office within 48 hours to add a credit-card to your account.

Cancellation policy *

I agree that my account will be charged a $15.00 cancellation fee if I cancel my reservation less than 24 hours before my scheduled appointment
