I understand that I will need to call in a credit-card within 48 hours of receiving my confirmation email.

You will receive an email confirmation with your scheduled appointment date within one business day after receiving this online registration form. Once you have received your confirmation email from us with your scheduled date of service, please call our office within 48 hours to add a credit-card to your account.
